Na podstawie art. 65 ustawy zdnia 14 lipca 2006 r. dziny obywatela Unii Europejskiej do∏àcza si´ pi´ç o wjeêdzie na terytorium Rzeczypospolitej Polskiej, aktualnych fotografii osoby, której wniosek doty- pobycie oraz wyjeêdzie z tego terytorium obywateli czy; paƒstw cz∏onkowskich Unii Europejskiej i cz∏onków ich rodzin (Dz. U. Nr 144, poz. 1043) zarzàdza si´, co 2) wymian´ dokumentu potwierdzajàcego prawo sta- nast´puje: ∏ego pobytu lub karty sta∏ego pobytu cz∏onka ro- dziny obywatela Unii Europejskiej do∏àcza si´ § 1. Rozporzàdzenie okreÊla: dwie aktualne fotografie osoby, której wniosek do- tyczy. 1) wzory formularzy wniosków o: 2. 1. Wniosek dotyczy tylko jednej osoby. The application refers to only one person. Demande ne concerne qu'une personne.

2. Należy wypełnić wszystkie wymagane rubryki. W przypadku braku niezbędnych danych wniosek może być pozostawiony bez rozpoznania. AII required fields should be filled in. If necessary data are missed the application may be left unprocessed. II faut remplir toutes les cases exigćes. En cas d' absence des donnćes nćcessaires, la demande peut rester sans instruction.

3. Formularz należy wypełnić czytelnie, drukowanymi literami wpisywanymi w odpowiednie kratki. The application should be filled in eligibly, in capital letters entered into the appropriate boxes. Le formulaire doit tre rempli lisiblement, en majuscules inscrites dans les cases correspondantes.

4. Części A, B, C, Di E wypełnia wnioskodawca. Parts A, B, C, D and E should be filled in by the applicant. Les parties A, B, C, D et E sont a remplir par le demandeur.

5. Część F wypełnia organ przyjmujący wniosek. Part F shall be filled in by the authority receiving the application. La partie F est a remplir par 1 autoritć recevant la demande.

6. Część G wypełnia organ rozpatrujący wniosek. Part G shall be filled in by the authority processing the application. La partie G est a remplir par I'autoritć chargće de I instruction de la demande.

7. W części A w rubryce „płeć” wpisać M - dla mężczyzny, K - dla kobiety; w rubryce „stan cywilny” należy użyć sformułowań: panna, kawaler, mężatka, żonaty, rozwiedziona, rozwiedziony, wdowa, wdowiec, wolny, wolna.

In part A in the „sex” field „„M” should be entered for a male and „„K” for women; in „„marital status” fields the following terms should be used: unmarried, married, divorced, widow(er), single. Dans la partie A, a la case „„sexe” - inscrire „„M” pour homme, „„K” pour femme; rubrique „,situation de famille”, utilisez une des formulations suivantes: cćlibataire, mariće, marić, divorcće, divorcć, veuve, veuf, libre.

1. Wniosek dotyczy członka rodziny obywatela państwa członkowskiego Unii Europejskiej, obywatela państwa Europejskiego Obszaru Gospodarczego nienależącego do Unii Europejskiej lub obywatela państwa niebędącego stroną umowy o Europejskim Obszarze Gospodarczym, który może korzystać ze swobody przepływu osób na podstawie umów zawartych przez to państwo ze Wspólnotą Europejską i jej państwami członkowskimi, jeżeli członek rodziny nie posiada obywatelstwa Unii Europejskiej lub ww. państw.

The application concerns a family member of a citizen of a European Union Member State, citizen of a non-EU European Economic Area country or citizen of a state which is not a party of the agreement on the European Economic Area but who may use the freedom of movement of persons based on the agreements concluded by this state with the European Community and its Member States, if the family member is not a citizen of the European Union or the above-mentioned states.

La demande concerne un membre de la famille d'un ressortissant de 1 Union europćenne, d'un ressortissant d'un Ktat de I" Espace ćconomique europóen non membre de I"Union europóenne ou d'un ressortissant d'un Etat n'etant pas une des parties de 1'accord sur I'Espace ćconomique europćen, qui peut bćnćficier de la libre circulation des personnes en vertu des accords conclus entre cet Ktat d'une et la Communautć europćenne et ses Etats membres d'autre part, lorsque cet membre de la famille n'est pas ressortissant de lI'Union europćenne ou n'a pas la nationalitć des Etats visós ci-dessus.

2. Wniosek dotyczy tylko jednej osoby. The application refers to only one person. Demande ne concerne qu'une personne.

3. Należy wypełnić wszystkie wymagane rubryki. W przypadku braku niezbędnych danych wniosek może być pozostawiony bez rozpoznania. AII required fields should be filled in. If necessary data are missed the application may be left unprocessed. II faut remplir toutes les cases exigćes. En cas d' absence des donnćes nócessaires, la demande peut rester sans instruction.

4. Formularz należy wypełnić czytelnie, drukowanymi literami wpisywanymi w odpowiednie kratki. The application should be filled in eligibly, in capital letters entered into the appropriate boxes. Le formulaire doit €tre rempli lisiblement, en majuscules inscrites dans les cases correspondantes.

5. Części A, B, C, Di E wypełnia wnioskodawca. Parts A, B, C, D and E should be filled in by the applicant. Les parties A, B, C, D et E sont a remplir par le demandeur.

6. Część F wypełnia organ przyjmujący wniosek. Part F shall be filled in by the authority receiving the application. La partie F est a remplir par l'autoritć recevant la demande.
